What is OCD therapy?

Obsessive-compulsive disorder (OCD) is a mental health condition involving unwanted, intrusive thoughts, images, or urges (obsessions) and repetitive behaviours or mental rituals (compulsions) that are performed to reduce distress or prevent something feared from happening. While OCD is often associated with excessive cleaning or checking, it can take many different forms. 

Common presentations include contamination fears, intrusive thoughts about causing harm, relationship doubts (sometimes called relationship OCD or ROCD), religious or moral scrupulosity, a need for symmetry or things feeling “just right,” and repeated attempts to gain certainty or reassurance. OCD can also involve primarily mental compulsions, such as reviewing memories, analyzing thoughts, mentally checking, or repeatedly seeking reassurance. Because OCD can look very different from one person to another, some people experience symptoms for years without realizing that OCD may be the underlying cause.

The first-line, evidence-based treatment for OCD is Exposure and Response Prevention (ERP), a specialized form of cognitive behavioural therapy (CBT). In ERP, a therapist helps you gradually approach situations, thoughts, sensations, or uncertainties that trigger OCD while reducing or resisting the compulsive behaviours you would normally use to feel safe or certain. With practice, this helps break the cycle of obsession and compulsion and build greater tolerance for uncertainty and distress. Because ERP requires specialized knowledge and training, it's worth looking for a therapist with specific experience treating OCD rather than assuming that any CBT therapist provides ERP. OCD treatment can also incorporate approaches such as Acceptance and Commitment Therapy (ACT), which can help you respond more mindfully to intrusive thoughts rather than trying to eliminate or suppress them.

What to know about OCD treatment in Ontario

In-person and online OCD therapy

In-person OCD therapy is more widely available in larger Ontario communities, including Toronto and Ottawa, while online therapy can connect you with specialized OCD therapists anywhere in the province. Online therapy can be just as effective as in-person treatment and offer greater flexibility, reduce travel time, and make it easier to practise exposure exercises in your everyday environment. Neither format is inherently better. It all comes down to your preferences. 

Ontario credentials for OCD therapists

Ontario's regulated mental health professionals include Registered Psychotherapists (RPs), regulated by the College of Registered Psychotherapists of Ontario (CRPO); Registered Social Workers (RSWs), regulated by the Ontario College of Social Workers and Social Service Workers (OCSWSSW); and Registered Psychologists, regulated by the College of Psychologists of Ontario.

For OCD treatment, however, a therapist's professional designation is only one factor to consider. Look for someone who has specific training or clinical experience in ERP and OCD treatment. A therapist with extensive experience treating OCD may be a better fit than a therapist who has the right designation but does not regularly use ERP.

Session cost and insurance coverage

Private therapy sessions in Ontario typically cost $120–$230 per session, though fees vary with the therapist's credentials, experience, and location. Many extended health insurance plans provide some coverage for psychotherapy or psychological services, but the amount and eligible provider designations depend on your specific plan. Some plans may cover services from Registered Psychotherapists, Registered Social Workers, or Psychologists, while others may have different requirements or annual limits.  Before booking, check your benefits plan to confirm these details. How to choose an OCD therapist in Ontario

1. Look for explicit ERP training

General CBT experience isn't necessarily the same as training in Exposure and Response Prevention (ERP). When comparing therapists, look for specific mention of ERP, OCD treatment, or exposure-based therapy in their profile. You can also ask about their training and how regularly they use ERP with clients.

2. Consider their experience with your OCD symptoms

OCD can take many forms, and a therapist who regularly treats OCD will be familiar with a wide range of presentations, including contamination fears, harm-related intrusive thoughts, relationship OCD, sexual or religious obsessions, scrupulosity, and “just right” experiences. You don't necessarily need a therapist who specializes in your exact OCD theme, but it can help to choose someone who is comfortable treating the symptoms you're experiencing.

3. Ask how they approach exposure work

A qualified ERP therapist should be able to explain how exposure exercises are developed collaboratively and tailored to your symptoms. Exposure typically involves gradually approaching triggers while learning to resist the compulsive responses that maintain the OCD cycle. A good therapist won't overwhelm you or force you to confront your fears before you're ready. They'll follow your pace. 

4. Ask about treatment pacing

ERP can involve regular practice between sessions, and some people benefit from more frequent sessions, particularly during certain stages of treatment. Ask a prospective therapist how they typically structure OCD treatment, how often they recommend meeting, and what they expect clients to practise between sessions.

What's the difference between CBT and ERP for OCD?

ERP, or Exposure and Response Prevention, is a specialized form of CBT developed specifically for OCD. While general CBT may be helpful for anxiety and other mental health concerns, ERP focuses on gradually facing situations, thoughts, or sensations that trigger OCD while learning to resist the compulsions that maintain the cycle. ERP is the most established psychological treatment for OCD.

Can OCD be treated effectively online in Ontario?

Yes. Online ERP is an established treatment option for OCD, and exposure exercises can be conducted effectively online with a therapist who has appropriate training. Online therapy can also make it easier to practise exposures in your everyday environment. For many people in Ontario, online treatment provides access to OCD specialists who may not be available in their local community.

Are OCD therapists in Ontario covered by insurance?

It depends on your specific extended health plan and the therapist's professional designation. Many Ontario plans provide coverage for services from Registered Psychotherapists (RPs), Registered Social Workers (RSWs), or Registered Psychologists, but eligible providers, annual limits, and reimbursement amounts vary between plans. How long does ERP treatment typically take?

There's no single timeline for OCD treatment. The length of ERP depends on factors such as the severity and type of OCD, how frequently you attend therapy, how much you practise between sessions, and your individual response to treatment. Some people experience meaningful improvement within a relatively short course of structured ERP, while others benefit from longer-term treatment or additional support.

Does OCD always involve visible behaviours like handwashing or checking?

No. Compulsions can be mental as well as physical. Some people experience primarily mental compulsions, such as reviewing memories, mentally checking, analyzing intrusive thoughts, silently seeking reassurance, or repeatedly trying to achieve certainty. The term “Pure O” is sometimes used to describe this presentation, although mental compulsions are still compulsions and are an important target of ERP.

Is there in-person OCD treatment available across Ontario?

In-person OCD therapists are more concentrated in larger Ontario communities, including Toronto and Ottawa, although availability varies. If there isn't an ERP-trained therapist nearby, online therapy can provide access to specialized OCD treatment from elsewhere in Ontario.
